Health Forms and Immunizations
International students must have a health history and immunization record on file at Missouri S&T Student Health Services.
All students are required to submit a completed Health History Form and Tuberculosis Screening Form for New Students. The forms and additional information about new student medical requirements are available online.
Students attending Missouri S&T are required by federal law to be current on vaccinations for measles, mumps, and rubella (MMR). You must provide proof of two MMR shots, and both must be after the age of 1 year.
If students will live in campus housing, they must provide proof of the meningitis vaccine, which must be given after age of 16 years.
If you answered YES to any of the questions on the Tuberculosis Screening Form, then you will need to be tested for tuberculosis (blood test). Please note the following:
- Quantiferon Gold TB test (QFT) or T-Spot are the only TB tests that are accepted
- TB tests are only valid within 3 months before arrival
- TB Skin test is NOT accepted
If at all possible, we recommend you obtain a QFT before coming to the United States. Please note it can be done no earlier than 3 months prior to coming to school. If you have the QFT test in your home country, please bring official documentation for submission to Student Health Services. This document must be from the performing laboratory, including your name, date test completed, and test results.
Students who have not had the QFT test within 3 months of arriving in the United States or who do not have the required documentation will be required to have a QFT test within the first semester of attending Missouri S&T. Students are encouraged to have this test completed during orientation week. Students who are covered by the health insurance available through Missouri S&T will be charged $50 for the QFT test. This reduced amount has been negotiated by the university and the laboratory who processes the tests. This charge will be placed on your student account and you will be responsible for payment of this cost.
It may be more cost effective and convenient for you to have each of the above requirements done before your arrival to campus, so students are encouraged to get the required and recommended vaccinations prior to leaving your home country. Please bring documentation showing the vaccinations you have received for submission to Student Health Services. Students who do not meet the vaccination requirements may receive the vaccinations from Student Health Services at a reduced cost. Students are responsible for all costs associated with mandatory or recommended vaccinations.
